SORRY. ALL VACCINES ARE NOW ALLOCATED.

CHAW PCN has prided itself on never wasting a drop of vaccine. Due to a slight oversupply of vaccine, we may have a limited supply of spare Pfizer vaccine this afternoon only (Thursday, 13th May 2021).
If you are available at short notice today and wish to receive your 1st dose of Pfizer vaccine, please get in touch using the link at the page below. Priority will be given to the following groups of people:
- Anyone who still needs a second dose of Pfizer vaccine (if 1st dose administered already by CHAW PCN)
- Immunocompromised or clinically vulnerable patients
- Pregnant ladies
- Patients aged 38 years or above
- Patients registered with practices in Chelford, Handforth, Alderley Edge and Wilmslow
Please note that as this is our last anticipated delivery of Pfizer vaccine, future second doses will not be provided at CHAW PCN sites but the local Clinical Commissioning Group has given assurances that second doses will be arranged at other sites within 12 weeks of the 1st dose. Please note that CHAW PCN cannot take any responsibility for any patients not receiving their second dose of Pfizer vaccine in such circumstances.
If you wish to receive a spare dose of Pfizer vaccine today, please leave your details, using the link below, including:
- NHS Number (if possible)
- Date of Birth
- Gender
- First name and last name
- Postcode
- Telephone number
This page will be updated as soon as all spare doses of Pfizer vaccine have been allocated. Due to the limited number of spare Pfizer doses, we anticipate that some patients will be left disappointed after applying for a vaccination and we apologise in advance. Please do not turn up to the surgery without a confirmed appointment for a vaccination. We have no doubt that the vaccination programme is progressing well and that everyone eligible will receive their vaccination in due course. Please do not contact surgeries by phone as they are already extremely busy handling normal medical queries.
